Spontaneous proximal LAD dissection

 

Teaching

Spontaneous coronary artery dissection (SCAD)

  • Rare cause of ACS
  • Majority diagnosed postmortem
  • Associated with young, healthy females (mean age 35-40 yrs)
    • 1/3 patients are peripartum
    • Peak incidence 2 weeks post-partum
  • Other causes: atherosclerosis, connective tissue disorder
  • Proximal LAD most common in women
  • RCA most common in men
  • No clear definitive treatment
